With Physician Clinic Patient Portal, you can view your personal physician office health record whenever and wherever you have access to the Internet.
The best part of the Physician Clinic Patient Portal, it’s FREE! All of these features are free for Portage Health and Upper Great Lakes Family Health Center patients, and are securely housed within the hospital’s electronic medical record system.
The Physician Clinic Patient Portal is NOT meant for urgent care requests or emergencies.
Urgent appointments, including appointments needed within the next 3 business days, should be requested by telephone. Please call your primary care provider's office.

You never know when it will be important to have access to your health record, so we’ve made it available to you 24-hours-a-day, 7-days-a-week. Take a quick peak at your records, or download them and print them to look at later.
On your first login, your information might look a little sparse. Don’t worry, once you have your next appointment, more information will be added.
Don’t remember what the doctor recommended at the end of your visit? Now you can log into the Patient Portal and view a visit summary for your past office visits.
We will post your next doctor’s appointment right on your Patient Portal so you’ll be able to look it up anytime. You can see all your scheduled future doctor’s appointments in one place. We will also continue to remind you!
Do you want to take another look at your lab results? Within this feature, review non-sensitive lab results.
Not all lab results will be posted immediately; many of them need to be approved by your healthcare provider before being listed on the Patient Portal. Often times your physician won’t make them available until after consulting with you about the results.
While we perform about 95 percent of lab tests at the Portage Health facility in Hancock, we do send some tests out to other facilities. Those tests will take longer to get results.
When it’s time to refill your medication, login and send a message to us through the secure Patient Portal. Use the form provided to send the name of the medication and your preferred pharmacy.
We will review your request and let you know if we have any questions or concerns.
All prescription requests made on the Patient Portal will be sent to your preferred pharmacy within 2 business days. If we have questions about your request, we will call the phone number you provide in the request form.

We can also let you know about preventative health screenings and immunizations that are recommended for you.
Through the Patient Portal, you will be able to send secure messages to clinic staff members. We will be able to respond to your message securely and the message will arrive in your Patient Portal inbox.
