    At Portage Health, we constantly strive to provide you with a team of health professionals to give you the best care possible, right here at home. By working as a team to put you in charge of your care, we have adopted the principles of a patient-centered medical home facility. We are continuing to work to provide you with the same great care while addressing your individual needs. Please read on to learn more about patient-centered medical home and how to be a better participant in your care.

    We will help you make decisions for your health, and we will respect the decisions you make.
    We will explain tests and results with you, making sure you understand everything about your health.
    Every one of us, from receptionist to provider, will keep your information confidential.
    We will help you live a healthy and long life by seeing you as a whole person with physical and psychological needs.
    We will give you a visit summary and a list of current medications.


    We’ll Make Time For You

    When you call for an appointment, sharing the reason with us will help ensure you have the right type of appointment. You and your provider need enough time to cover all of your concerns. If we can’t fit you in with your regular provider, we’ll offer you a visit with another provider, who will make sure your regular provider knows about your visit.
    To fit your busy schedule, we also have Express Care, with convenient hours 365 days a year. Express Care has access to your medical information, will update your medical record and notify us of your visit within 24 hours.
    If you can’t keep an appointment with us, please call and let us know 24 hours in advance.


    How Can You Help?

    Remember the saying, “An ounce of prevention is worth a pound of cure.”

    Come to see us for an annual visit even if you are well. We can help you stay that way. If you have any ongoing medical problems like high blood pressure, asthma or diabetes, don’t wait to see us until something is wrong. Talk to us about how often a planned visit should be made to check on your condition. Make your next appointment before leaving our office. Bring all of your drugs and supplements, or an accurate list, with you to every visit. We need to know what you are taking so that we can be sure you are safe and being treated properly.
