    Our Social Services department helps connect patients and their families with the community resources and information they need to deal with an illness. We offer:

    • Community resource information and referrals 
    • No eligibility requirements 
    • Services free of charge

    We also offer referral assistance to: 

    • Home nursing (private duty and skilled care) 
    • Lifeline
    • Senior Meals
    • Housekeeping assistance
    • Transportation
    • Durable medical equipment
    • Financial assistance
    • Medicare/Medicaid application assistance
    • Financial charity programs
    • Durable Power of Attorney for Health Care
    • Guardianship
    • Support groups
    • Nursing home placement

    Portage Health social workers are available:

    Monday – Friday
    8 a.m. – 4:30 p.m.

    Referrals to social services can be made by a physician, nurse, family member or individual. Referrals can be made to the social worker by phone, pager or by stopping in the office, which is located on the second floor of Portage Health’s main campus.
