    Home Care provides a variety of Medicare-certified, hospital-based health care services to patients in their homes. Chances are, there have been times when an illness or surgery has derailed your health and put you on the sidelines. These are the times when we can help.

    Our philosophy is that every patient should receive care in the safest, most appropriate and least restrictive setting. We are a vital link in a chain of Portage Health services, all working together to help you recover in your own home. As a Medicare-certified, Joint Commission-accredited agency, we provide some of the highest quality care in the Copper Country.

    What does Home Care offer?

    • Registered Nurse Case Managers who coordinate all home care services. They make skilled nursing assessments, provide direct care, teach self care and instruct family members in the care of their home-bound loved ones.
    • Speech Therapists who evaluate and treat speech, communications and swallowing disorders.
    • Physical Therapists who assess and treat functional impairments that result from injury, stroke, surgery, fractures or other medical conditions.
    • Occupational Therapists who develop and supervise individualized programs that restore functional skills such as bathing, eating, writing and other daily living activities.
    • Home Health Aides who provide personal care and assist with treatments under the direction of the registered nurse.
    • Medical Social Workers who assess social and emotional factors that influence the outcome of home care, and assist in resolving any problems.
    • Telehealth Services, which can transmit a patient\'s vital signs and other perinent information to the Portage Health Home Care office. There a nurse can immediately review it and contact the doctor with any concerns.
    • Infant Phototherapy, which is available to families of newborns who might otherwise need to stay in the hospital longer to undergo light therapy for jaundice.
